How does it work if you get certifications from multiple certification agencies? Do you get a card from each one that lists the specs you have from each?

For instance if I'm PADI certified OW, and want to get a Nitrox or Drysuit certification through a different agency, do I get a card from the new agency that says I'm OW and the new cert, or do they strictly list the certs you have gotten through them?

Yes, you get a separate card from each agency and it only lists the certification for the course you took. For example I have a NAUI open water card, a PADI AOW card, and a TDI Nitrox card. Hope this answers your questions.
